1. What is important to know upon preparing for nose surgery?
In this regard, there are two issues that should be paid attention to:
1. What will the new nose look like? Please note that the new acquired shape depends on the preoperative shape of the nose, the thickness of the skin, whether the nose was injured, the state of health of the body, etc. All the marked items must be clarified before the operation.
2. Another important thing to know is how healthy your new nose will be after surgery, which is a very serious question. This is described in more detail in question 3.
2. What kind of candidate are you for nose surgery? What will be the result?
During the rhinoplasty operation, the change occurs only in the cartilaginous and bony parts of the nose, while the skin of the nose remains the same and adapts to the new nose over time. The thickness of the skin, its elasticity, etc., is one of the main factors determining the final result obtained.
1. Thin skin - If the nose is not very large, and the skin is thin, such patients are usually waiting for the desired result, after the operation, the nose takes its new shape in a short period of time.
2. Medium-thick skin: such patients will get a good, sometimes beautiful result, but the adaptation of the skin to a new shape will take longer compared to thin skin. (From 1 month to 1 year).
3. Too thick skin and injured nostrils: if the nose skin is too thick and large before the operation, then the result on such noses may not turn out as we would like. Depending on the severity of the injury, certain changes occur in the anatomy of the nose, so it is considered a complex nose that requires special practice from a doctor. Injured, with various bends, the nose may contain certain asymmetries. (Skin memory).
For example, if the tip of the nose is sharply shifted to the right or left as a result of an injury, then after the operation, there will most likely be a difference in the holes. Too large, thick-skinned and injured noses belong to a difficult category. Of course, for a perfect result, in addition to the patient's skin, it is necessary to take into account his age, facial features, his own characteristics of the body, and most importantly-the choice of a highly qualified doctor. 3. Why should you give preference to the method used by Dr. Gorkhmaz Azimov?
Rhinoplasty of recent years has one motto - protective rhinoplasty. Studies around the world regarding the problems faced by people who have undergone surgery in old age show that the situation here is not very good. Thus, the most famous surgeons in the world note the appearance of postoperative problems in about 10 % of patients. About 5% of these patients undergo severe repeated nasal surgery before reaching old age, requiring rib cartilage surgery. That is, such an operation is necessary for 50 out of 1000 patients who have undergone nose surgery. But if we look closely, we will see that the main reason for heavy repeated operations that require the removal of rib cartilage is that the dorsum of the nose was cut during the operation. The method used by Dr. Gorkhmaz Azimov and published in the journal of the Association of Plastic Surgeons of the United States is considered a leader in its protective properties. The more the anatomy of the nose is preserved during the operation, the fewer problems there are in the subsequent period.
4. What does open and closed rhinoplasty mean? Other methods of rhinoplasty.
During the operation, access to the nose can be opened or closed.
1. With closed access, incisions are made inside the nostrils.
2. With the open method, the same two incisions are also made, but an additional visible incision is made in the middle, connecting these incisions.
This incision allows the doctor to work comfortably, completely removing the skin of the nose. So what do we lose with this small cut? During this incision, 2 nerves, 2 small arteries and a ligament that supports the tip of the nose are cut out. As a result, such patients have more swelling in the nasal part after surgery, there is a violation of sensitivity. In some patients, the incision site does not look beautiful, mainly on" snub-nosed " noses. With closed and open equipment, the same work is preserved, but the closed process is performed in more inconvenient conditions. And this requires additional skills from the doctor. Our choice is mainly closed rhinoplasty. While piezorinoplasty or ultrasound rhinoplasty, another method that you have heard about today is not considered a method. Because the piezo is just one of the devices used for cutting bones, and it does not affect the final result.
5. Why it is important to preserve the dorsum of the nose in time. Dorsal preservation - what is it?
The process of rhinoplasty surgery can be divided into 2 stages;
1. First of all, the cartilage at the tip of the nose is given a shape, while the tip of the nose is raised, thinned, the nose is shortened, etc.
2. At the second stage, the height of the dorsum (hump, bump) decreases. This procedure is performed in 2 different ways, which are very different.
Traditional methods a beautiful side view of the nose is given by removing the nasal dorsum, then the remaining wide nasal bones are cut from the lower part and come closer, as a result of which the spinal part of the nose becomes thinner.
But, unfortunately, such an incision of the spinal section significantly weakens the bone-cartilaginous "frame" of which the nose consists. This is the main cause of problems that occur after nose surgeries, because the cut cartilage does not join together like bones.
The result is problems with nasal breathing (sniffing, snoring) associated with the nasal valve, which occur when the cartilage tissue weakens, various deformities of the dorsum (subsidence, curvature, etc.)
In countries where rhinoplasty has been used for a long time (for example, in the United States), the progression of cartilage tissue as people who have undergone rhinoplasty surgery age, forces such patients to repeat surgery. The treatment is exclusively repeated surgery on the nose.
Unfortunately, in some cases, there are severe operations that require the removal of rib cartilage.
But if the anatomy of the spinal part is preserved during the operation, then such problems do not bother you, since the cartilage and bones of the nose retain their fullness, and, of course, the healing time will also be a very short period. In operated patients, during the operation, you can manually check whether the dorsum is preserved.
The sensations when touching, softness, smoothness should be the same as in an inoperable nose, even after 1 month.
That is, the phrase "the nose can not be touched for 6 months after the operation" does not apply to our method.. (link to the scientific work).
Every year in Azerbaijan alone, about 1,000 patients undergo repeated operations to remove rib cartilage due to complications associated with the incision of the dorsum during surgery, and this figure is increasing every year. At the same time, certain problems with the nose are solved, but complications can still occur with age. While using our new method (since the end of 2018), such problems do not arise, since the anatomy of the new nose remains in the same shape as it was before the operation, that is, it is completely preserved.
